I was trying to find the slide that showed the failures that led to the Boeing 737 disasters. It showed how a culture of implied or express threats caused people to not report issues/concerns. I thought it might have been in @allspaw's presentation but I didn't see that in the downloaded slides. Or maybe in @tlimoncelli's presentation? Anyone remember?
