5 whys is a bit accusatory by nature? Should we use ”how come” instead of ”why” ;) we use the 5W technique regularly
in my experience, “why” is sometimes a proxy question for “who” — in an attempt to assign blame. doesn’t promote honesty, sharing, and learning when people feel there might be negative consequences in the answer.
We experimented with using "5 how's" but I also found that if the answer is already a "who" e.g. "Dave made a mistake" then asking "why" a few times might lead you to a systemic rather than human cause.
You could also try the 5 who's or 5 what were you thinking methods 😉
Yes the 5 wtfs method is something that often takes place on an incident bridge

@chankramath, I’d love to learn more about how CCF relates to cloud costs. One would assume a fairly linear relationship. How could I learn more about the topic of GreenOps in general, do you have any reports or a white paper?
