For Psychological-Safety, I have run management book-club where we read Sidney Dekker books “Drift Into Failure” and “Just Culture” this helped them look at how we impact our teams when trying to find root cause and looking to not make it a person and more a system and support approach… We also work with engineer involved treating them more like a “2nd victim” where they get a chance to redeem themselves and help other avoid the issue, even having them seem more like role of “breaker” someone that has knowledge on how and why something breaks…
